Our services 

We provide housing and support in Walton-on-Thames, Molesey, Hersham, Thames Ditton, Hinchley Wood, and Cobham.

Our client groups 

Currently we support around 65 clients with complex needs. Our clients include individuals who are homeless, some with drug and/or alcohol issues or mental health challenges and young people.

What makes our team unique

Trust in each other is an integral part of our team dynamic. We are a little work family, which means we all support and watch out for each other’s health and wellbeing. Our team is full of humour, smiles and positive energy, and this helps us build professional relationships with clients. 

We’re also very good at problem-solving together when unexpected situations arise, which happens a lot because we work with very complex cases. We always make sure we carry out a debrief following a challenging situation and look at where we can improve practice and performance in the future.

Our team’s biggest success

We are very proud of the trust we have built with clients, as they know we are always here and available to support them. It’s a huge privilege to work with clients who share some of their most vulnerable and intimate stories with us – it’s what makes working in the Elmbridge team worthwhile.

Over the years we’ve also learned to work in harmony with other services and take a partnered approach when needed. This helps our clients to engage with us and leads to more positive outcomes for them, which is fantastic!

On Friday, 30 August, Transform is proud to participate in Starts at Home Day 2024 – an event dedicated to highlighting the value of support services and the profound impact having a safe and secure place to call home can make to people’s lives.
The team at Pound House have received high praise from an Independent Reviewing Officer (IRO) at Surrey County Council (SCC). IROs chair Child Looked After Review meetings where they advocate for the young person and hold the professionals around them to account.
