Either fill out a form at your local store or you can select your local store and nominate Transform online, it’s that easy!
Waitrose
Visit your local Waitrose store and request a Community Matters application form. All you need to do is provide some brief details and hand the form back to a member of staff.
The few minutes it takes to complete a form could result in Transform becoming one of the three charities chosen in that store.
Tesco
Nominate Transform in store by requesting and completing a form or submit a form online. Customers vote via the blue token they receive at the checkout
and three charities will be given £1000, £2000 or £4000!
